不对,你可以这样问:

what happened?

what was happening?

what happened?
what was happening?

一般不这么说，是what happened? 发生什么事了。表示过去，但没有被动的意思，所以前面不加was

错。happen不用被动的，
what happened?发生了什么？
what's happening?正在发生什么？
what will happen?将会发生什么？
what's happened?发生过什么？

。。。要有was,只有这样what was happening?过去进行时。反正不用被动啦。

至于怎么回答，根据它问题的时态回答。
what happened?发生了什么？过去时就答过去时——He died.

不对啊，happen不能用于被动语态

不能用was
happen是发生的意思 不能用被动语态。
用was的话，就可以说：what was going on?
what was taking place? (take place也是发生的意思)
