老师说最前面的那个All of the city's hospitals是个比喻句，意思是整个城市像个医院。我实在无法理解为什么啊

你不理很正常，因为老师讲解有误。

上面三个结构分明是并列的名词结构来做主语的，第一个中间的all相当于100%。
老师说的意思应该如此表达：
All the city was like a hospital.
like"像……一样”。
老师说的不对,是:这个城市的所有医院,75%的工厂和建筑物,90%的住宅.....
根据上下文也应该知道老师说的不对.
"师不必贤于弟子".我们做弟子的也要经常对老师的观点与自己的想法作比较,有怀疑的地方要进行商榷,而不是一味的盲从.这才是现代的师生关系.
